Welcome to your medical home!  Patient-centered is a way of saying that you, the patient, are the most important person in the health care system.  You are at the center of your health care.  It is a way for you to be informed about and involved in your health care decisions.  We will work together to ensure all of your preventative, acute, and chronic care needs are met.

You will select your primary physician, who will help you manage your total health care.  Your physician will direct the care team (clinical staff) to coordinate your care.  They will keep in touch with specialists to make sure you get the care you need.  They will make sure testing is completed, results obtained, and you are notified of your results in a timely manner.  They will track your health information using electronic medical records and offer you the option of using e-mail through the patient portal via the Internet to keep in touch with them.  If you are admitted to the hospital, the team will make sure the physician seeing you in the hospital has your health information.  When you are discharged, your care team will review your hospital stay and make sure you return for follow-up.

You can help by being an active team player.  Talk with your team about your health questions.  Tell your team about other health care professionals who care for you and ask any specialists you see to send reports to your primary care physician, including your eye doctor.  Follow the health care plan you and your team have worked out and make sure that you understand it. We will work with you to help you self-manage your care by developing a treatment plan with clear goals and steps to reach them.  Once you begin to see results, you and your team can discuss adding new goals.  If you are having trouble sticking with your care plan, let your team know.  You can also call the clinic for advice before going to the Emergency Room in a non-emergent situation.

Great news! We are now able to prescribe Via gra to our patients.
